Phase-Lost Slateback
The Phase-Lost Slateback is the reward for the achievement Phase-Lost-and-Found, which requires obtaining all Phase-Lost weapons from Phase Diving in K'aresh.
Phase-Diving orbs require you to be in phase diving and have the Reshii Wraps at rank 3 or higher. You have about a 20% chance of obtaining a transmog appearance when collecting an orb. You cannot get appearances you already own. You can check this comment with more information about good routes to run for collecting all appearances.
