BlizzardCS has started writing comments on Wowhead quest pages, noting that certain quests are bugged and that they're investigating the issue.
Escort Quests
BlizzardCS has commented on the quest pages of Freedom to Ruul, Free From the Hold, and The Defias Brotherhood. These are all escort quests that can immediately fail if the quest giver dies or, in this case, despawns.
Blizzard
We're aware that Ruul/Gilthares Firebough/Defias Traitor may disappear when moving near or across zone and area boundaries, causing the quest to fail. We're already investigating the issue.
Undead Paladin Quest
One of the early Undead Paladin quests is Rediscovering the Light, which requires players to rediscover how to use Holy Light. Sometimes healing the NPCs doesn't give progress towards the quest objective and Blizzard is aware of the issue.
BlizzardWe're aware that healing Injured Deathguards may not grant progress and we're already investigating the issue.
