Mythic L'ura
Mythic L'ura is around a 300+ pull boss for most guilds, with the majority of wipes (~60%) occurring in Phase 1. As so many wipes happen in phase 1 on progression, it's frustrating when you wipe after that, especially when you wipe to a bug that is completely out of your control.
There are some bugs on this encounter that still exist and while rare, will usually wipe guilds at least once on their progression.
Phase 2 Jerk
The common strategy in Phase 2 is to have 4 cores get shot by Galvanize in an X pattern. This creates a pull effect that sucks all players into the center of the arena. On Mythic difficulty, shortly after this pull effect, the entire raid must spread due to an AoE centered on every player. In addition, since this deals cosmic damage, all crystals are dropped in the center and the raid spreads appropriately.
However, there's a bug that happens very rarely where the pull effect seems to stutter a lot. This causes players to be jerked around inconsistently causing players to get pulled to a non-central location and when players try to drop their crystals, they get dropped not in the center, very likely wiping the raid.
Phase 3 Crystal
At the start of Phase 3, players are knocked out of the Darkwell alongside the crystals. There are two soaks that land in the nearby area that need to be soaked.
For some reason, a crystal will occasionally not fly the full distance out of the Darkwell and remain inside it. There is no recovery from this and it is guaranteed to wipe your raid.
Phase 3 Light Aura
Early on in Patch 12.0.5, there was a bug where the Light Aura will consistently not apply to players. This was "fixed", but can still occur in Phase 3. If this happens, the target is slowed massively so they need a snare break similar to Paladin's Freedom and need to leave the light and come back to maybe have it trigger correctly.
